3-Dimensional approach to anti-aging

On May 21, Dr Sonia Tekchandani, Consultant Dermatologist, Tender Skin International – India and UAE, joined Aesthetic Medicine live to deliver a session on ‘3-Dimensional approach to anti-ageing’.

Here’s listing select key takeaways from the discussion:

– Skin health and beauty represent overall ‘well-being’.

– What is a golden ratio: A mathematical ratio of 1.618:1, and the number 1.618 is called ‘Phi’.

– The oval face shape is the perfect shape. The proportions of the length of the nose, the position of the eyes and the length of the chin, all conform to some aspect of the Golden Ratio.

Measurements of the Golden Ratio

– Measure the length and width of the face and divide the length by the width. The ideal result – as defined by the golden ratio – is roughly 1.6, which means a beautiful person’s face is about 1 ½ times longer than it is wide.
– Measures three segments of the face – from the forehead hairline to a spot between the eyes, from between the eyes to the bottom of the nose, and from the bottom of the nose to the bottom of the chin. If the numbers are equal, a person is considered more beautiful.
– The length of an ear is equal to the length of the nose, and the width of an eye is equal to the distance between the eyes.

Triangle of youth

– Three unique features: High cheekbones, full cheek volume, and a well-defined jawline.
– As we age, this youthful triangle often becomes inverted. The face becomes more oblong, cheeks flatten and jowls appear.

Aging: It is multifaceted. It is called senescence.

Signs of aging in upper third of the face

– Transverse forehead rhytids
– Glabellar frown lines on forehead
– Drooping eyelids
– Flattening and lowering of eyebrows
– Sunken temples
– Brow ridge prominence

Signs of aging in the middle third of the face
– Crows feet and bunny lines
– Obvious tear trough
– Sunken cheeks and cheekbone area
– Saggy cheek fat
– Deep smile lines (nasolabial folds)

Signs of aging in the lower third of the face
– Minimal vertical rhytids
– Development of ornamental grooves – lines at corner of the mouth
– Buccomandibular crease at the side of cheek and above the lower jaw
– Pronounced Jowls with sagging chin (fat deposition before jawline)

Perioral aging: A dropping appearance of the perioral area is a characteristic sign of aging.

Principals of 3D skin rejuvenation: Resurfacing, relax muscle, refill, and resuspend

Good anti-aging treatment for the face
– Upper 1/3 face: Botulinum toxin and fillers for forehead and temples
– Middle 1/3 face: Fillers, energy-based device and threads
– Lower 1/3: Botulinum toxin for masseter, platysma, smoker’s lines, filler for pre-jowl sulcus and threads

In conclusion
– Filling in zones rather than individual lines helps to accomplish better cosmetic results.
– Classically, Botulinum Toxin restores the upper face whereas fillers, threads and energy-based devices help to restore the lower face.
– However, combination of procedures on a case-to-case basis results in synergy and a 3-dimension rejuvenation.
